Enter the prices for eyeglass components, select the patient's poverty level, and calculate the final cost based on the sliding scale policy.

How It Works
- Enter Prices: Fill in the retail prices for the Frame, Lenses, and any Materials/Add-ons.
- Answer Questions: Use the radio buttons to specify if polycarbonate is included and if the patient meets medical criteria for a discount.
- Select Poverty Level: Choose the patient's corresponding Federal Poverty Guideline level.
- Discounts Applied: For patients under 200% FPG, a discount of $50 off frames and $110 off lenses is automatically applied. A medical necessity discount of $80 for polycarbonate is also applied if applicable.
- Charge Back Added: A specific charge back amount is added to the discounted total based on the selected FPG level.
- Calculate: Press the 'Calculate Total' button to see the final price the patient is responsible for.
